Account changes

Please refer to the service forms listed below for information on how to complete these transactions.

To change a legal name, address of record, or beneficiary/Indirect Participant, complete the appropriate Account/Certificate Change form.
To assign an Account or Certificate as collateral, complete the appropriate Collateral Assignment form.
To change the firm and/or the Sales Representative on the account, please complete the appropriate Change of Firm or Sales Representative form.
To authorize a Third Party to direct the company to accept and act upon instructions, please complete the appropriate Third Party Authorization form.
